Swollen Joint is unlocked after completing either of the achievements Good Old Times or Most Popular Idol.

On the Injury's card, you can choose to 'prick it with a needle'. This will remove Swollen Joint but also lower your Stamina by 2 and Maximum Stamina by 1.
