"You should get rid of it"
This is a magical equipment item (Slot C) with a big trade off. Depending on how far your Doom level is, this can be fantastic as it will remove 10% of Doom from the playthrough. However, if you are conscious about your Reason, the removal of -3 to its Max can be risky.

The Gruesome Totem is unlocked after completing the World Doomed III achievement.

It can be bought at the Occult Shop for 3 FNDs afterwards.
